Free & Reduced Price Lunch Forms
Please be sure to complete and return the Free and Reduced-Price Lunch form that was sent home with your child this fall. Even with school meals being provided for free to all students, these forms help our district receive funding for programs.

Celebration Packs
Do you have an upcoming birthday or special classroom event? Celebration packs are here to help!
To request a celebration pack, fill out the form below and submit it along with payment to your student's teacher. You can order chocolate chip cookies, sugar cookies, yellow cupcakes with frosting, chocolate cupcakes with frosting, and milk. Cookies are $0.50 each, cupcakes are $0.75 each, and milk is $0.50 each.
Payments must be made in cash or a check. Checks can be made out to "Hammond Cafeteria". Orders must be received one week prior to the event.
